- Fixed term to 31 December 2018, with a possible extension to contract
- 40 hours per week (1.0 FTE)

You will coordinate access and provide brief interventions and packages of care for 12 years onward who experience mild to moderate impact on their wellbeing following mental health and/or addiction issues.
This is a great opportunity to join a dedicated and committed team. We want you to apply if you:
- Are a registered health professional with a current practicing certificate
- Have at least two years’ experience in Community Mental Health service delivery
- Are proactive, self-starting and responds and adjusts to changing work demands and circumstances
