This position is responsible and accountable for the day to day management of the Care Coordination Team. The Service Manager will oversee allocation of referrals to Care Coordinators, ensure quality care planning and coordination of services to clients.
 
To be successful in this role you will have:
  • Tertiary qualifications in Social or Health Science, Registered or Enrolled Nurses with current registration.
  • Demonstrated experience in team leadership, financial management and service marketing.
  • Knowledge of Home Care Packages, HACC and other community services in the Aged Care Sector.
  • Knowledge of community standards e.g. Community Care Common Standards, Department of Human Services Standards
  • Well-developed communication, planning, organisational and time management skills.
  • Experience in assessment, case management and service delivery to people who are aged and disabled living in the community.
  • High level problem solving and analytical skills.
  • Ability to develop and maintain professional relationships with a broad range of internal and external stakeholders.
  • Ability to work independently and as part of a team to achieve organisational objectives.
  • Excellent computer skills, knowledge of Carelink Plus would be an advantage.
Desirable:
  • Experience with brokerage model of service, Individual Support Packages and Consumer Directed Care.
  • Post graduate qualification relevant to Care Management, Health or Aged Care.
