The Facility

This is a Not-for-Profit organisation whom pride themselves on being able to provide quality care and support to the elderly. As the Clinical Care Coordinator/Manager, you will be working in a modern large facility providing both high and low care. This team works hard to provide an atmosphere where the residents are comfortable and can feel at home.
 
The Role

As acting Clinical Care Coordinator (CCC) your key duty will be to supervise Clinical Care within the establishment in conjunction with ensuring policies, procedures & ACFI/Accreditation processes are adhered to.
  
You will be the main contact for clients; implementing, monitoring and modifying in-home care plans to match the need of clients and manage risk.
 
You will work alongside and be supported by the Care Service Manager to ensure that quality care is delivered in accordance with our client’s standards to provide a holistic person cantered approach to care.

The Person

The ideal candidate for this position will have the following:

• The client requires a Registered Nurse (Div1) with current AHPRA registration and Current Police Check
• The person will have excellent clinical documentation practice
• The person will ensure high standards of care are provided in line with Policy, Procedures and accreditation standards
• Have a thorough understanding of the Aged Care Funding Instrument (ACFI) 
• Have the ability to lead and implement change within the facility
