Lead Practitioner – Support Facilitator

Partners in Recovery (PIR) are looking for a Support Facilitator to join their team in Nunawading. This is an exciting opportunity to work alongside people experiencing mental ill health and through the provision of referrals and service co-ordination work towards achieving their individual recovery objectives. PIR is a supportive team that fosters a culture of best practice through provision of educational and collaborative processes. The PIR program is transitioning to the NDIS, with the focus of the transition phase to support the NDIS roll-out.
In this role you will:
- work full time or part time, up to 38 hours per week on a fixed term contract until 30 June 2019
- provide support, information and advice to people with mental ill health and their families and carers
- assess individual client needs using PIR assessment tools to identify and engage appropriate service system supports
- working collaboratively with clients and their care and support teams to develop and implement shared PIR Action Plans
- assist clients to apply for and gain access to the NDIS or help them find alternative supports
You will have:
- tertiary qualifications (minimum certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy or other health related field as designated by Mind
- knowledge, values and practice experience that aligns with Mind’s Recovery Model of practice
- an understanding of and ideally experience with the National Disability Insurance Scheme (NDIS)
- flexibility and adaptability to work within an evolving service sector
