About the opportunity
The HARP program is a partnership between Uniting Vic.Tas and Eastern Health. The program sits in the psychosocial stream of Eastern Health’s HARP and works closely with clinicians of other HARP streams - Diabetes, Cardiac, Respiratory and Nursing - based at Box Hill, Maroondah and the Angliss Hospitals, Wantirna Health and the Peter James Centre. Case Managers have access to specialist geriatrician, neuropsychology and psychiatry clinics and to a range of allied health clinicians.
Our clients are frequently disadvantaged due to poor health literacy and lifestyle choices and have often disengaged from social and health management norms. Clients live in the Eastern Health catchment area, which includes Whitehorse, Maroondah, Knox, Yarra Ranges and parts of Monash, Manningham and Boroondara.
We are flexible, innovative and inclusive in our approaches to care and problem resolution, and seek to empower clients and carers through the provision of short-term case management support to:
- Access long-term services such as home care, personal care and social inclusion.
- Access carer supports including respite care and counselling services.
- Liaise with Centrelink, the National Disability Insurance Scheme and My Aged Care to assist clients to navigate complex application and assessment processes.
- Liaise with Eastern Health ambulatory and inpatient services.
- Liaise with community-based mental health and alcohol withdrawal services.
- Engage with and strengthen communication with GPs.
- Explore individual and non-conforming situations.
