Exciting opportunities have become available for Registered Nurses (Care Managers) to join our collaborative and supportive team at Bolton Clarke in the Eastern suburbs of Melbourne.

These two roles will service our clients specifically in the following areas:

• Mount Waverley, Burwood & Ashwood
• Prahran, South Yarra, Toorak 

The purpose of this role is to promote and provide quality service to clients,act as a resource person and manage staff through clinical activities. This is a permanent full time opportunity and will involve traveling to client’s homes. 


 Your role:

• Undertake admissions as allocated
• Coordinate the care of clients and their carer/s
• Manage the care of clients and their carers
• Engage in collaborative practice to achieve client outcomes with peers and teammembers
• Act to enhance the professional development of self and others such asGraduates, Novices and Students
• Function in accordance with legislation and common law affecting nursing practice
• Provide services in accordance with funding guidelines for project initiatives

What do we need from you?

• Sound clinical knowledge and experience
• An innovative and dynamic approach to the provision of care
• Well-developed problem solving skills particularly when dealing with complexcare scenario’s
• Effective persuasion and influencing skills
• Ability to employ an education and development approach when assistingemployees
• Excellent communication skills both written and verbal
• Excellent organisational and time management skills
• Proven ability to contribute to and, work as part of a dynamic team in additionto operating independently

Essential requirements:

• Registered Nurse (Division 1) Qualifications
• Current and satisfactory National Police Record check
• Current nursing registration with AHPRA
• Current Victorian Drivers Licence

Nice to have:

• Experience in a similar role within Aged Care or similar field
