On behalf of our well known client, KLC Recruitment are currently seeking an experienced Case Manager to work with an established and new group of clients. Ideally you will have prior experience working within the aged care sector as either a client advisor or case manager.

To be successful in this role you must have:

• Tertiary qualification or 6+ years of relevant experience in the health and welfare discipline
• An understanding of the community care service and the needs of its clients
• Demonstrated experience in person-centred practises and complex case management
• A Current Working With Childrens Check and hold or be willing to undergo a National Police Check 
• A current and valid Drivers License 

Your responsibilities in this role will include:

• Providing specialised, comprehensive and coordinated case management and support to persons who are frail aged and/or with a disability
• Tailoring  packages of care to assist individuals and their families to live in the community
• Initial in home visits conducting assessments of the client
• Advocate and liaise with other service providers on behalf of your client
• Maintaining client budgets within set parameters, negotiating and providing brokered service provision for clients and quality improvement activities.
