The Opportunity

The Medibank at Home Rehabilitation at Home program is dedicated to providing a hospital substitute option for Medibank members undergoing specific surgical conditions to undergo rehabilitation in their home, instead of inpatient rehabilitation at the hospital.

As our Care Coordinator within the Medibank at Home team, you will have overall responsibility for delivering the assessment and development of coordinated rehabilitation plans for Medibank members accessing the Rehabilitation in the Home program. You will also be responsible for the education, outcome monitoring and engagement of Medibank members in the program.
Key responsibilities will include:

• Effectively applying inclusionary and exclusionary criteria, in conjunction with appropriate clinical judgement, to determine a member’s appropriateness for rehabilitation in the home;
• Understanding the health and social benefits for Medibank member of providing rehabilitation in the home and to be able to communicate those benefits in an engaging, sensitive way;
• Demonstrating a high level of knowledge in the area of orthopaedic rehabilitation and discharge planning and clinical and non-clinical risks assessment, stratification and management;
• Ensuring rehabilitation plan recommendations are appropriate and tailored to a member’s needs, and considerate of all risks identified; and
• Understanding the key drivers for service success for each of our external and internal stakeholders, and tailor communication and engagement accordingly.

Please note we have multiple roles available for a passionate & result oriented Care Coordinator to join are Medibank at Home team in our offices at Brisbane.

About You

To be successful in this role, you will present as a Registered Nurse (Div 1) with previous experience across general medical and surgical acute care. A strong background across pre-admission, discharge planning, transitional care programs and/or rehabilitation in the home programs will also be highly sought after. You will also have and maintain full AHPRA registration.

You are a clear & concise communicator with the ability to think clearly and pragmatically. Experience in using IT systems for recording and monitoring of patient activity, as well as, an ability to effectively manage and prioritise your work duties, will be key drivers of your success. Most importantly, you will be passionate about influencing positive change in healthcare and be confident in your presentation and phone manner.
