Programmed Health Professionals/Clinical Care Professionals are seeking experienced Registered Nurses with the skills and passion required to maintain independence and improve health outcomes for DVA clients living in the community setting by providing and facilitating safe, responsive nursing care.

You will provide professional and clinical support to our DVA gold card holder’s, ensuring they receive comprehensive and holistic care to continue their independence and enable them to remain in their place of living longer.

In this role you will ensure that each client receives comprehensive assessment; liaising with allied health services and auxiliary services to ensure continuity of care and mitigate hospitalisation.

Duties include:

  • Care plan and clinical assessment management.
  • Medication management including IVs
  • Wound management
  • Stomal therapy
  • Management of chronic, complex medical condition
  • Management of palliative care and life limiting conditions.
  • Post-surgery management

Essential Criteria:

  • Minimum 2-3 years’ experience working in a disability or community setting.
  • Use of own vehicle, driver’s license, car registration and insurance must be sighted.
  • Current unrestricted AHPRA Registration
  • Great communication skills
  • 2 clinical referees
  • Current 1st aid certificate
  • Current Working With Children’s Check
  • Strong client focus in regards to advocacy and reporting details.
  • Solution driven with a can do attitude!

Desirable Criteria:

  • Previous experience with the processes and protocols related with DVA community support work
  • Advanced clinical expertise
