About the company:
Vivir Healthcare are seeking Social Workers’ with experience in Aged Care Assessments for Aged Providers. These are excellent opportunities for Social Workers’ with Aged Care experience who are looking for a new challenge.
About the role:
The role involves conducting comprehensive assessments of frail older people and to facilitate access to available care services appropriate to the clients care needs as per the Aged Care Assessments Program Guidelines.
The responsibilities of the role include:
• Assessment of care plans as per Aged Care Assessment Guidelines
• Working with an inter-disciplinary Allied Health Team to help improve the lives of elderly clients
• Provide clinical services to patients and guidance to junior staff
• Documentation and tracking of funding related options
To be eligible for this position, you must be:
• An experienced Social Worker with eligibility to AASW
• Experience and knowledge of Aged Care Assessment Accreditation standards
