About the business and the role
All Queanbeyan GP Super Clinic Practice Nurses report to the Nurse Manager and will undertake treatment and clinical duties as required by the general practitioners, clinical processes and deliverables to achieve objectives in line with the QGPSC business plan.
The practice nurses have the responsibility for undertaking and assisting GP's, allied health and other team members with clinical duties and workload. Ensuring a professional clinical environment, team coordination and efficient management of patient treatment, focusing on patient care.
QGPSC is an accredited practice and has an integrated approach to patient care, with 21 general practitioners, a nurse team, allied health team and visiting specialists. An administration and management team support the delivery of care.
The practice is co-located at the Queanbeyan GP Super Clinic with radiology, pharmacy, pathology and dental services
Key Tasks and Duties:
- Change bed linen
- Restock doctors rooms
- Check temperature vaccines fridge & record
- ECG’s
- Vaccinations & immunisations
- Blood pressure checks
- Ear irrigating
- Procedure preparation & management
- Clean & pack instruments
- Sterilisation
- Health Assessments
- GPMP’s, TCA’s
- Restocking of theatre & clinical area
- Recall letters & act on recalls for doctors (i.e. phone patient)
- Triage patients
- Liaise with hospital & community health where necessary
Key Responsibilities
Clinical Practice
In associations with general practitioners & allied health:
- Assess, plan, develop, implement & evaluate programs to promote health & wellbeing, & prevent adverse effects on health & wellbeing
- Implement & evaluate individual treatment plans for patients with a known long-term condition or illness
- Identify, & manage as appropriate treatment plans for patients at risk of developing long term conditions.
- Support patients to adopt health promotion strategies that encourage the patient to live healthy
- Assess and care for patients presenting with uncomplicated wounds
- Implement and participate in vaccination & immunisation programmes for both adults & children.
- Advise, support and where appropriate administer vaccinations for patients travelling abroad
- Assist senior practitioners in providing minor-surgery sessions
Examples: INR’s, health assessments, wound management, ECG’s, immunisations and provide the patient some information on healthy eating.
Communication
- Demonstrate sensitive communication techniques to ensure patients are fully informed and consent to their treatment
- Support patients dealing with bad news or issues regarding their health
- Utilise communication skills to encourage and support patients with their prescribed treatment plans
- Effectively communicate with GP’s and co workers
- Ensure confidentiality
- Ensure patient notes are updated promptly when treated
Examples: Update patient notes; communicate with GP’s regarding patients, health assessments.
Service Delivery
- Recognise and own within your own competence & professional code of conduct.
- Request assistance where own competence is not suitable
- Produce accurate & complete records of patient treatment & consultation
- Prioritise, organise and manage own workload in a manner that maintains and promotes quality care
- Assess effectiveness of care delivery through self & peer review, benchmarking and formal evaluation.
- Participate in the maintenance of quality governance systems & processes across the organisation & its activities
- Evaluate the patients response to health care provision and advise the GP accordingly
- Participate in the ongoing management of clinical care, learning & identifying improvements through patient complaints, clinical incidents, and near misses.
Examples: Patient records, Opportunity for improvements, health assessments
Team Work
- Support and participate in shared learning across the practice & wider organisation
- Accept delegation from other nurses, prioritise own workload and ensure effective time management strategies
- Work effectively with other nurses and the wider team
- Participate in team activities that create opportunities to improve care.
Examples: clinical meetings, delegation of tasks and duties, nurse team meetings
Risk management
- Follow internal policies and procedures
- Participate in local implementation strategies that are aligned with values and culture of general practice
- Apply infection control measures within the practice according to local & national guidelines
- Ensure safe storage, rotation & disposal of vaccines and drugs is undertaken.
- Oversee monitoring, stock control and documentation of controlled drug use in accordance with legal requirements
- Monitor work areas and practices to ensure they are safe and free from hazards and conform to policies and procedures
- Ensure all team members are supported and assisted where necessary
Examples: fridge procedure, infection control procedure
Learning & Development
- Disseminate learning and information gained to other team members in order to share information
- Assess own learning needs and undertake learning as appropriate
- Make effective use of learning opportunities within and outside the workplace, evaluating their effectiveness and feedback relevant information
- Act as a mentor to students, assessing competence and providing guidance
- Provide knowledge, information & experience to others in the team for cross team learning & development
Examples: attend training sessions, provide feedback to other nurses
Quality
- Review and request updates to policies, procedures and systems where appropriate
- Manage and assist in accreditation requirements where necessary
- Assist & report all incidents, accidents and near misses immediately to practice manager
- Assist with reporting to DoHA, general practice networks in relation to clinical matters and so forth where appropriate.
- Adhere to all clinic policies and procedures in relation to OHS&E
Qualifications:
- Qualification and registration as a Registered Nurse
Accreditation / Affiliation / Associations / Registrations
- Registration
- May be a member of the Nurses association
Experience:
- Previous experience working within teams in a medical environment or general practice environment
- Excellent communication skills and be able to communicate with all levels of personnel, ethnic backgrounds and ages.
- Extensive experience managing various clinical requirements and programs
- Experience in managing large workloads and various tasks and priorities
- Previous experience and knowledge in chronic disease management and enhanced primary care models and strategies
- Experience in completing health assessments, GPMP’s and TCA’s
- Comfortable handling large volumes of patients and various treatments
