THE POSITION

You will be an experienced ACFI Coordinator keen to join us in this new role to lead the ACFI team in maximising funding outcomes for the benefits of our residents. Ensuring the standardisation of ACFI processes and procedures for all Residential Aged Care Services, you will report to the Director of Care and work alongside the management team. Your effective communication skills will establish positive relationships with all staff and residents.  You will work directly work with care staff to gather, assess and collate detailed information to accurately assess residents and take a prominent role in their ACFI education.

SELECTION CRITERIA

  • Commitment to work in harmony with Catholic Values and Principles
  • Registered Nurse, endorsed by AHPRA with evidence of recent practice in aged care (essential)
  • High level understanding of the ACFI instrument in Residential Aged Care facilities
  • Practical experience of ACFI to include knowledge of competency in Medicare Online and ACFI submission
  • Working knowledge of relevant Aged Care Legislation, the regulatory framework, agency accreditation standards and quality standards
  • Ability to assess, plan, implement, evaluate and document resident's care and clinical care requirements
  • Strong leadership skills with coaching and mentoring experience
  • Demonstrated time management and organisational skills
  • Effective decision making skills with ability to apply initiative and flexibility, and be accountable for actions
  • Understanding and participation in the continuous quality improvement process
  • Ability to professionally liaise with colleagues, residents, families, and allied health personnel
  • Ability to work as part of a team, as well as accept responsibility and work unsupervised
