Permanent Part Time 64 hrs/Fortnight
An excellent opportunity has arisen to become a member of our high achieving, client centred team with the Transition Care (TCP)/GEM@ Resi program. The program is seeking a dynamic and experienced health professional from an allied health (Grade 2) or nursing background (Division 1) to fulfil the role as TCP/GEM@Resi Care Coordinator.
Transition Care provides case management and care coordination from hospital to community care settings, and this role can encompass all streams of Transition Care (Home based and Bed Based) and GEM@Resi.
Assessment and care planning skills, commitment to teamwork and the ability to work in both the hospital and community settings are required. Excellent communication skills, including the capacity to negotiate effectively, the ability to work in a fast paced environment and to establish priorities are essential components of the role.
The Transition Care/GEM@ Resi Team is a friendly and dedicated team who support each other to provide optimal care for aged clients following a hospital stay. Working as part of a multidisciplinary team is required. Supervision and professional development opportunities are provided.
