Purpose of Position

War Memorial Hospital (WMH) is a 3rd Schedule Aged Rehabilitation hospital, situated in Waverley, owned and operated by Uniting and part of the South Eastern Sydney Local Health District (SESLHD). Services are comprised of 24 hour inpatient care, Monday to Friday outpatient departments, several specialist community teams inclusive of Transitional Aged Care, Aged Care Assessment Team, Geriatric Flying Squad, Young Onset Dementia, 7 day a week day centre, 24 hour accommodation service for regional guests and the full range of hospital support services. It is set within beautiful tranquil grounds and is a well-supported team environment.

The Aged Care Nurse Practitioner (NP) / Clinical Nurse Consultant (CNC) is an integral member of the Geriatric Flying Squad (GFS), co-ordinating and delivering high quality nursing care to patients/clients in our community. The NP / CNC provides advanced clinical assessment, consultation, referral pathways and development of management plans including (where appropriate) hospital avoidance strategies for residents of the local community and Residential Aged Care Facilities (RACF). Based in the northern area of the South Eastern Sydney Local Health District, the position also provides extended care hours for the current Geriatric Flying Squad during weekdays and weekends to RACF's.

The extended practice role includes comprehensive assessment, interpretation of diagnostics, implementation of treatment modalities and relevant referral skills applied to a broad range of challenging, complex and changing situations within the SESLHD community and RACF setting. The Advanced Practice Nurse will work autonomously and in collaboration with interdisciplinary teams, seeking support and advice from the Geriatricians on-call where appropriate.

Referrals may be received from and made to community based services including, but not limited to, General Practitioners (GPs), RACF’s, ASET teams, Geriatricians, Emergency Departments, Community Care Teams, RACF Managers and nurses, and other specialty services. The position utilises existing pathways and establishes new pathways to ensure provision of care will be delivered in the setting most appropriate for the client.

The Advanced Practice Nurse will incorporate education provision to RACF staff with a strong emphasis on linking education with current clinical issues. Education provision will also be pro-active and strategically planned for a diverse range of geriatric conditions that affect wellness within a defined and agreed scope of practice and clinical guidelines.

This position is 0.5 - 0.7FTE with negotiable days & times across weekdays, after-hours and weekends.

Selection Criteria

1. Tertiary qualification at Masters level and endorsement to practice as a Registered Nurse or Nurse Practitioner with AHPRA (or working towards same). 2. Current class C drivers licence. 3. Minimum 3 years fulltime equivalent experience within aged care or critical care. 4. Minimum 5 years fulltime equivalent post-registration experience. 5. Demonstrated advanced skills in the comprehensive assessment and management of the older person. 6. Demonstrated leadership abilities within the specialty and the profession, including delivering educational material. 7. Demonstrated high level problem solving and decision making skills including written and verbal communication skills. 8. Demonstrated ability to undertake clinical service planning.
