The Role:

Southern Plus is looking for an experienced Registered Nurse with a passion for supporting clients in a community setting. The role of Wellness Partner (RN) combines the need for clinical expertise with case management to achieve holistic care of high care clients in their home environment.

The key responsibilities of this role are to work collaboratively with clients, their representatives and external supporting agencies to assess, plan and facilitate the delivery of care that best meets the client’s needs. You will support the client to manage their chronic and complex disease process, taking into account the clients goals, budgets and the level of control clients have over the delivery of care. You will provide a high quality care service while supporting and encouraging the client to achieve their optimum independence and engagement with life.

 

The Candidate:

The ideal candidate is an experience and hard-working Registered Nurse with a passion for providing high quality service to all of our valued clients and their families. With the ability to foster positive relationships with clients and colleagues, the ideal candidate will exhibit a ‘can-do’ attitude and bring positivity and light to the team. Candidates must also fulfil the essential criteria below:

·         Current registration with AHPRA (RN)

·         Current ‘C’ class WA drivers licence

·         Experience supporting clients to manage their care in a community setting

·         Previous experience working in aged care and/or home care environment

·         Contemporary, up-to-date clinical skills and knowledge of industry best practice principles

·         Demonstrated cultural awareness

·         Exceptional organisational, time management skills and ability to prioritise tasks

·         Knowledge of Microsoft Office programs and an aptitude to learn new systems
