Uniting is a NDIS LAC Partner delivering NDIS Local Area Coordination services in NSW.


About LAC Transition Services


The Local Area Coordination (LAC) Transition Services is rolling out an important aspect of the National Disability Insurance Scheme (NDIS). The aim of the programme is to empower people with disabilities and their families and carers to build goals, awareness and inclusivity enabling them to direct and control their decisions within their lives.

Uniting is seeking Local Area Coordinators across the following 5 Districts:

  • Nepean/Blue Mountains
  • Western Sydney
  • Illawarra Shoalhaven
  • Northern Sydney
  • Southern NSW


Responsibilities

LAC’s will be responsible for working with participants, their families and carers, and other parties to encourage change in societal beliefs and structures. They do this by:

  • Providing support to prospective NDIS participants to engage with the Scheme.
  • Assisting participants to implement their plans, including building community inclusion and capacity.
  • Complete plan reviews and full scheme plans for participants.


Are you the person we are looking for?
Essential requirements
You will be an active community member who is enthusiastic about supporting and empowering people with disability and their families. We are looking for people who have lived experience, or awareness of the impact of existing or acquired disability upon individuals, families, carers and the community.

A minimum of three years relevant experience and/or relevant degree or diploma in allied health, disability services, education, human services, community development, social work and local government or other customer facing environments that support or provide services to the community.

Desirable requirements

Experience in a fast paced, target driven environment (such as Disability Employment Services) whilst helping people in your community is highly desirable.
