Chandler Macleod are currently looking for an experienced Case Manager (Home Care Packages Program) to work for our client based in Dandenong to commence immediately in a 4-day-a-week permanent role with a competitive salary on offer of $70K- $75K plus super depending on experience (pro rata)

**To be considered for this role you MUST have previous experience working as a Case Manager - Home Care Packages Program (HCPP)

The purpose of the position is to engage with clients to identify their service goals and ensure optimum access for clients to live they life they choose with the support of a Home Care Package.

The role requires the successful applicant to use a person centred, strength-based service approach to develop and implement a care plan and service budget, make referrals to internal and external services; monitor and review service goals and promote our clients services. The Case Manager provides a central point of contact for clients receiving Home Care packages and their carers and families.

Context

Our client exists to support people who are blind or have low vision to live the life they choose. They provide services to more than 32,000 clients from 28 sites in NSW, ACT, Queensland and Victoria and 29 clinics as well as outreach services to the Northern Territory and Tasmania.

Principal Responsibilities

• Using a person centred, strength-based service approach, plan, develop and coordinate services for the client through the intake assessment process and liaise with internal and external services to achieve the desired outcomes for clients;

• Monitor and review service plans and goals in consultation with clients on an ongoing basis. Provide support and assistance as appropriate for relevant parties;

• Act as the Primary Service Provider to coordinate the provision of services internally for clients and families and liaise with external service providers to achieve desired outcomes for clients;

• Identify, develop and review information and resources; promote and represent Vision Australia regarding Case Management in relevant forums;

• Develop and maintain internal and external networks;

• Provide professional development as appropriate to relevant parties.

• Monitor and maintain accurate, up to date client records in accordance with professional standards, our clients policy and procedures and other relevant legislation including Disability Act.

• Undertake continuous professional development including keeping up to date with new research, maintain current knowledge re the HCPP and maintaining skills & knowledge level;

• Maintain a safe workplace by ensuring staff, volunteers and clients receive adequate information, training and instruction on WHS. Provide adequate supervision to all persons on Vision Australia sites and events and ensure all safety issues are reported and addressed.

Essential Job Competencies

• Degree in relevant Human Services field or equivalent with a minimum of 2 years’ recent experience in the delivery of Home Care Packages.

• Knowledge and skills in working with older adults, their carers and families.

• Demonstrated experience in providing client support and case management services.

• Strong planning skills including sound time management skills.

• Well-developed analytical and problem solving skills.

• Ability to work as part of a team and independently as required.

• Well developed skills in establishing and building positive working relationships at all levels.

• Excellent interpersonal and written communication skills.

• Computer literacy skills in Microsoft applications, the internet, client management and finance systems.

• Empathy and emotional maturity.

• Ability to adapt to changes in the work environment.

• Experience working with families from culturally and linguistically diverse backgrounds (desirable).

• Current membership of the Case Management Society of Australia (desirable)
