What’s the Job?
There is a great opportunity for a motivated and enthusiastic individual to join our Eastern team as a Case Manager. The Case Manager role will work closely with the people we support, their families, carers and our experienced annecto team to assist people to remain connected within their community.

What we need from you:

  • Understanding and commitment to implementing Consumer Directed Care Initiatives (CDC) and Person Centred approaches
  • Knowledge of community resources/professional services which support individuals within the Eastern Metropolitan Region
  • Demonstrated ability to work effectively with a culturally diverse group
  • Demonstrated ability to work autonomously with a high degree of initiative
  • Excellent communication skills, both written and verbal 
  • Demonstrated experience in working with people at all levels in a medium to large organisation with multiple sites
  • Relevant tertiary qualifications in health/social science
  • Current Victorian drivers licence
