About the Role

The CRSW role will be part of the Partners in Recovery Program. The PIR program aims to support consumers with a severe and persistent mental illness, who also have many needs, and their carers and families to access services and supports from multiple sectors.

The Partners in Recovery program has been identified as a National Disability Insurance Scheme (NDIS) transition program, and is working with consumers of the service to test eligibility and facilitate entry in to the scheme, to ensure long term sustainable access to required supports.

 

The NDIS is a person centred, market driven, social insurance scheme established to provide support for people who have a permanent or likely to be permanent disability for the whole of their lives. Its intent is to give individuals:

  • Empowerment to choose supports and services
  • Holistic person centred planning
  • Tailored individual funding packages and portability

By using a solutions focused approach, the worker will be working closely with consumers, medical practitioners and other community support providers to explain the purpose of the scheme, and also the types of supporting evidence which will be required in a clear and accessible way.

 

Core duties

  • Provide direct support and rehabilitation to consumers within their community
  • Work closely with Support Facilitators in the PIR program to obtain relevant supporting documentation, build comprehensive NDIS applications for submission to NDIA for assessment
  • Maintain records and resources
  • Engage consumers and develop trusting and professional relationships
  • Collect, collate and maintain data on consumer contact

 

What will you bring to the role?

  • Relevant experience in a similar role
  • Ability and willingness to work effectively as part of a team, to meet program goals and objectives.
  • Knowledge of complex mental health and drug and alcohol issues.
  • Highly developed written and verbal communication skills
  • Be a team player
  • Knowledge of the recovery model

  • Police check (refer to PD) – disclosable outcomes considered
