Home Support Coordinator

Support the Program Manager to ensure adequate assessment and case management for the provision of quality personal care, household and social support to older people with low level and complex care needs in their own home for the Community Home Support Program (CHSP) and Home Care Package Program (HCP). The emphasis is on promoting independence and supporting choices in care; in consultation with the client and their carer/family. Home based exercise is being incorporated into these packages as a service improvement initiative. Monitoring care and client needs, reviewing care plans and supporting care workers in the field are ongoing functions of the role.

 

Ensure that service provision and the activities of the Division are customer focussed and professionally and effectively conducted by contributing to the development of an integrated team approach and culture which is highly responsive to the needs of business partners and external clients.

This is a part time vacancy, working 64 hours per fortnight and salary will be adjusted according to actual hours worked.
