EXPERIENCED SUPPORT COORDINATOR REQUIRED!

This exciting opportunity is available with Southern Cross Support Services (SCSS), in our Bundaberg head office.

The Role:
This is a crucial role that will include the organisation of specialist care in accordance with the NDIA guidelines.

The ideal candidate will be required to assist with the implementation of Client NDIS Plans, to identify all the options required for NDIS participants that are within the parameters of their NDIS plan and work towards their goals.

You will budget out all supports required for the client and ensure there are covered under their NDIS plan.

You will be self-driven and work tirelessly in building participants capacity to be able to coordinate their own supports.

Your excellent communication and time management skills will see you liase with support providers and coordinate service agreements for each area of the participants NDIS plan whilst also working with Plan Managers to break down NDIS plan funds and how they are used.

Some travel to SCSS operation sites will be required in this role.

About SCSS:
SCSS is a licensed care provider that has offices in Bundaberg, Fraser Coast and Rockhampton and provides services in these areas together with Kingaroy, Emerald, Brisbane and Gold Coast. Our core business is to provide a range of community and family support services to meet the needs of those that are most vulnerable in our community.
  
With our industry entering the NDIS, the role of Support Coordinator will play a significant part in the way that SCSS will reach its goals and provide outstanding service to our clients. The role will be a full-time position on a 12 month contract.

Selection Criteria required by the applicant: 

The following criteria must be met for consideration for this position:
• Experience with the NDIS framework together with understanding of what it means for our Clients
• Certificate IV in Disability or combination of relevant experience, education and training equivalent to this qualification.
• Exceptional personal presentation with a professional manner for client relations.
• Exceptional multi-tasking, problem solving and decision-making skills.
• Exceptional attention to detail, time management, organisational and administrative skills.
• Excellent customer service skills and pleasant phone manner with an ability to remain calm, empathetic and assertive when faced with difficult callers.
• Excellent written and verbal communication skills

Desirable, but not essential.
• Strong knowledge of local disability community and providers
• Advanced computer skills, including the ability to use Microsoft Office, email and web based applications.
• Ability to work both independently but also within a collaborative team environment.
• Well-developed interpersonal skills with the ability to work productively with a broad range of people from a variety of backgrounds and experiences, including senior management, clients, colleagues and other stakeholders.
• Ability to achieve outcomes within agreed timeframes and under time pressures.
• Proven ability to work confidentially and productively with a high level of tact and diplomacy.
• Basic accounting and budgeting skills.
