Reporting to the Service Manager South East, you will partner with our clients to ensure the services they need and want are provided through effective, commercially-sustainable case management in line with the organisation's strategy, Christian Vision, Mission and Values. You will ensure that services are maximised and optimised ethically, by identifying client needs and matching the organisation's or brokered services and products to them. This position will be servicing clients who reside in the South West of Sydney (Campbelltown, Liverpool, Fairfield, Oran Park etc.).
To be successful in the role, you will have case management experience within a community, residential or hospital/acute care environment. You will be a resilient problem-solver who has the ability to cope with a high workload, who can work autonomously in a methodical way, and possess good awareness of the challenges that older people face. You will possess a relevant qualification in Aged Care, Community Services or Management and a Diploma in Case Management will be highly regarded. It is a requirement you have a current driver licence, comprehensively insured car and good driving record.
