Vision Australia is a leading national provider of blindness and low vision services in Australia. We work in partnership with Australians who are blind or have low vision to help them achieve the possibilities they choose in life.

We support more than 27,500 people of all ages and life stages, and circumstances. We do this through 28 Vision Australia centres in Victoria, New South Wales, the Australian Capital Territory, Queensland, South Australia and Western Australia; 30 clinics; and through outreach programs in the Northern Territory and Tasmania.

An exciting opportunity awaits an experienced Support Coordinator to join our team in Kooyong.

Responsibilities 

As an Support Coordinator at Vision Australia you 

• Using a person centred, strength-based service approach, plan, develop and coordinate services for clients across all funding streams  through a variety of stages in the service delivery journey; from initial contact and service planning to plan implementation and preparation for plan reviews
• Liaise with internal and external services to maximise participant outcomes;
• Mentor and support participants of NDIS, MAC and other individualised funding schemes to build their capacity to coordinate and implement their plans;
• Monitor and review service plans and goals in consultation with clients;

Requirements

To be successful in this role you will have:

• Degree in relevant Human Services with relevant work experience and specialist skills sufficient to perform these activities;
• Experience in developing networks with agencies such as community providers, community agencies, hospitals and Primary Health Networks;
• Knowledge and experience with funding programs and working to target. This includes negotiation, issue resolution and relationship development;
• Previous experience in working with people of all ages with disabilities in the community setting and providing client centred assessment and service delivery;
