This is a full time opportunity based in our Rockdale Office and you will report to the Team Leader in our Enabling and Life Building team.
The role is a 12 month position and Working hours are between 7.30am and 6pm, on a Monday to Sunday rotating roster.
Day to Day duties
· Collaborating with customers to develop individualised support plans that incorporate their identified wellbeing needs and goals
- Providing support to customers to improve their independence by assisting with living skills, transport and social activities as well as other support as required
- Supporting customers to explore their practical and complex needs
- Keeping customer records up to date on a daily basis using on line systems
- Developing and maintain professional relationships and networks with customers, families, caregivers and key stakeholders
- Seek out innovative ways to enhance the customer journey
What you need to have:
- Passion, Respect & Integrity
- Have lived experience as a consumer of mental health services and ability to openly utilise the personal recovery story to role model hope, choice, and self-determination.
- Knowledge of professional boundaries and the use of personal disclosure
- Self-motivated with the ability to work autonomously and in a team environment
- Ability and confidence to communicate and work with other staff outside of the immediate team
- Openness to feedback
- High level of accuracy and attention to detail
- Ability to multi-task and manage competing tasks while prioritizing self-care
- Understanding of Recovery Orientated Practice
