Job Description

Banyule Community Health is seeking to employ a Registered Nurse (Div 1) with experience in Asthma education to work with a small research  team based at Banyule Community Health , on an Behavioural Insights/ED Diversion Project .

The key responsibilities for this position include:

  • Conducts individual and family assessments to identify strengths, resources, psycho-social factors, educational needs and barriers to optimal health care and self-management.
  • Works with individuals with asthma, their families and healthcare professionals to develop, implement and revise the asthma action plan customized to individual needs, environment, disease severity and lifestyle.
  • Teaches the best use of medications and delivery devices, explaining technical concepts in understandable terms

The key skills & expertise required for this role include:

  • Substantial clinical nursing experience and skills in preventative health care and chronic disease management
  • Ability to work in a team environment 
  • Knowledge and skills in utilising a person centred approach to develop, implement and review the  Asthma Action  Plan
