About us:

Country & Outback Health's extensive primary health care footprint extends throughout regional and remote South Australia. We are looking for passionate professional to join our growing team to ensure we continue to deliver the best standard of care to our communities. These roles will work within our General Health Program and will be responsible for establishing and delivering services to rural and remote communities on the Eyre Peninsula to clients living with chronic disease.

About the roles

The primary function of this role is to build clinical skills, confidence and capacity within EP communities. You will achieve this through our collaborations with General Practice, and will improve client outcomes through applying your specialist skills to upskill Practice Nurses to integrate diabetes as a specialty within their practice.

You will have full support from our larger multi-disciplinary team to deliver best practice across multiple locations on the Eyre Peninsula.  

These are unique roles where you will be exposed to new experiences.

  • Your qualification as a Registered Nurse with a post graduate qualification in diabetes education, meets the criteria of our program. You will hold a current registration with AHPRA.
  • You will provide expert clinical care, assessment, education and self-management programmes to individuals with diabetes and their families or carers.
  • You will provide services within the general practice setting accelerating relationship building with the GP practice and other allied health professionals, resulting in more effective and co-ordinated care for clients.
