About the Organisation
Established on July 1st, 2015, the Gold Coast Primary Health Network (GCPHN) has the key objectives of increasing the efficiency and effectiveness of medical services for patients, particularly those at risk of poor health outcomes, and improving coordination of care to ensure patients receive the right care in the right place at the right time.
About the Opportunity
GCPHN has an exciting opportunity for a Senior Project Officer to join their team on a full-time contract basis until 30 December 2018, with the strong possibility of extension till June 2020.
As a Senior Project Officer, your ultimate responsibility will be to support the development of needs assessment and a regional plan for Palliative Care in the Gold Coast region.
Day-to-day duties will include (but will not be limited to):
- Undertaking and managing commissioning activities including planning and service co-design;
- Providing advice and working collaboratively with existing and potential stakeholders;
- Utilising a project management approach;
- Contributing to development plans for system change/redesign of program and projects in Palliative Care services, including focus on Residential Aged Care Facilities (RACF); and
- Monitoring contracts with consultants and reporting.
