ABOUT THE ROLE:
Mercy Palliative Care (MPC) is a community based Palliative Care Service in the Western region of Melbourne. MPC has a model of care where all disciplines — allied health, nursing, and medical work together as a team to meet the needs of each patient, their carer(s) and families. We offer holistic care in the home.
The Allied Health team responds to the diverse range of needs of patients and their families during times of loss and grief.
- 64 hours up to full time, permanent
- Psychosocial assessment and intervention to patients and families in their home
- Bereavement counselling and support
- Working closely with multi-disciplinary team and other service providers
- Supportive and friendly Allied Health team
- Rotates across three offices, Sunshine, Parkville and Werribee
ABOUT YOU:
- Recognised degree in Social Work (Eligibility for membership with AASW)
- Clinical skills and experience relevant to community palliative care context
- Excellent interpersonal and effective communication skills with high level of integrity
- Enthusiastic, flexible, dedicated, team orientated
- Current unrestricted drivers licence

Mercy Palliative Care is a community palliative care service operating in the Western region of Melbourne. Mercy Palliative Care provides a holistic multidisciplinary approach to palliative care. We provide psychological and spiritual support, caring for people of all ages with life-limiting illnesses in their homes as well as complex symptom management. We currently care for over 600 patients across the region in their own homes.
