UnitingCare Wesley Bowden (UCWB) is a Community Services organisation with an aspiration to be a leading provider of individual and community social services.  Its purpose is to deliver innovative, tailored services that support individual and community well-being.  Our focus is: vulnerable children and their families, older people, individuals who live with disability and their carers.

We are seeking an experienced Team Leader in our Individual Support Services Department who can provide service delivery coordination, undertake complex case work and provide leadership and supervision to a team of Support Coordinators.

This role assists with the delivery of quality, person-centred outcomes for our aged customers and customers living with disability which may include mental health issues and mental health challenges.

This role has the following key areas of responsibility:

  • Work as a senior member of the Ageing, Disability and Carer Support team and as a direct report  to the Manager, Individual Support Services
  • Maintain successful stakeholder relationships and assist the Business Manager to deliver service goals
  • Provide leadership and coaching to the team to ensure staff possess the required knowledge, ability, flexibility and drive to achieve quality outcomes for customers
  • Planning and directing team activities on a day to day basis through guidance and direction to staff on operational matters.
  • Triaging and allocation of customers to staff on the basis of assessed need
  • Provide direct supervision to staff including conducting case reviews and providing advice on complex cases
  • Provide direct supervision to staff including performance management and performance planning against the business strategy
  • Undertake complex assessments and case work for customers with specialist needs
  • Contribute to the development and delivery of new service offerings
  • Contribute to staff induction and ongoing training
  • Administer reporting requirements, monitor legislative standards and supervise the quality of data within the customer management system
