The organisation:
Not for Profit organisation that specialises in providing Community Aged Care Packages for people who are socially and financially disadvantaged.
The Position:
We are seeking an experience Senior Care Co-ordinator to provide leadership to a team of co-ordinators to ensure that service delivery targets are achieved, funding body requirements are met care and case management to consumers of the Community Aged Care and Veterans Homecare is coordinated and delivered.
Key Selection criteria:
- Relevant qualifications in aged care, welfare or related field
- Ability to supervision, lead and support CACP Coordinators
- Thorough understanding of CACP funding model
- A national police record check is required
- A current Victorian Drivers licence
- Excellent written and verbal communication skills
