We are seeking a sociable, enthusiastic community care professional to provide independent, ongoing advice and guidance to our Home Care Package clients in the Northern and Eastern regions of Melbourne. You will be working in a mobile team environment, developing and implementing care plans which are flexible, culturally appropriate and respect client choice.
About You
To be successful in the role you will be able to demonstrate:
- Capacity to work independently in a mobile environment as part of a multi-function team
- Ability to take the initiative, prioritise tasks and organise
- Excellent interpersonal communication, networking and negotiation skills
- Knowledge of Home Care Packages and Home Care standards, community resources and service networks for older Australians and their carers
- Experience with person centred care planning processes, including assessment and establishing client goals
- Experience managing client budgets, being responsive and accountable for time sensitive tasks
- Ability to work with set KPIs and achieve targets
- Ability to communicate expectations and maintain professional practice standards
- Related experience working in Home Care or Aged Care services
- Experience in the Microsoft Office suite and exposure and/or ability and confidence to acquire skills for in-house client database systems
Qualifications required for the role:
- Tertiary qualifications in social science, health/welfare discipline, or a related field
