Position: Isis Community Care Coordinator

Remuneration: $68,958
The Position:
The focus of this position is to coordinate the Home and Community Care Service Programs, related to the needs of the aged and younger people with a disability. Through the utilisation of specific carers the incumbent is responsible for facilitating access to services that fall within the identified needs and available financial resources.
Selection Criteria:
• Demonstrated ability to identify and respond to challenging needs in the Community Care environment
• Demonstrated experience in the area of Community and Human Services – Aged Care/Disabilities and or Frontline Management is desirable
• Qualifications in the area of Community and Human Services – Aged Care/Disabilities and or Frontline Management
• Ability to work unsupervised under limited direction and to work on several projects and initiatives at once and within agreed timeframes
• Demonstrated high level of interpersonal skills with a demonstrated ability to interact and communicate with all levels of staff, suppliers, public and government bodies
• Demonstrated ability to effectively plan, prioritise and set realistic goals for self and/or others to achieve agreed outcomes in a high Quality Community Care Program.
• Demonstrated very high level ability to prepare detailed reports and funding proposals and to manage Program finances and maintain current and accurate financial records
