About Us
The Salvation Army SalvoCare Eastern provides a range of Youth, Out of Home Care, Housing & Homeless, AOD and Chaplaincy services for statutory and non-statutory clients with complex needs throughout Eastern Metro, Peninsula and Gippsland areas.
The Housing Support Program assists young people, single adults and families with a holistic, case managed response to access housing and to address the issues that have contributed to their situation.
Position Details/Information
This position will provide case managed outreach and support to people who are homeless or at risk of homelessness to access safe, affordable accommodation and prevent their return to the homelessness service system.
The Case Manager will provide information, advice, advocacy, liaison and referrals to people who are assisted using a strength-based approach to identify goals to empower them to work towards establishing safe, secure, affordable housing.
Part of the position will also provide, when required, an Initial Assessment and Planning response to people who present as homeless or at risk of homelessness. Linkages into community supports and programs for clients with multiple and complex needs will be provided.
This position is located in Leongatha.
Hours and Award/Salary Level
This is a full time position, 38 hours per week fixed term to January 2019 (5 month appointment). Salary and conditions of employment are in accordance with Social, Community, Home Care and Disability Services Industry Award 2010, Social stream, Level 5.
What you can bring
- Relevant tertiary qualification in social work, community services, youth work, psychology or community development – diploma level (minimum).
- Relevant experience in a social service environment.
- Identifies and prioritises appropriate actions including referral.
- Knowledge in the areas of human development, health and family issues
- Expertise in working with clients with multiple vulnerabilities, histories of trauma, and complex needs
- Is non judgemental
- Looks for ways to exceed client expectations
- A willingness to undergo a Crimcheck is essential
- A current Employee Working With Children Check is essential
- A current Victorian Drivers Licence is essential
- Be able to provide proof of Eligibility to Work in Australia
