Our Home Nursing and Care Services team is seeking a Registered Nurse to undertake a dual role that includes assessment and home nursing duties. You will have experience working with older people, people with disabilities, and their carers, with the ability to assess their care requirements in the context of their psychosocial and functional needs.
  
To apply for this role it is essential you possess the following:
  • A tertiary qualification in a relevant discipline e.g. Nursing
  • Current AHPRA registration
  • Sound clinical and assessment skills;
  • Demonstrated experience in working with the frail aged and/or persons with a disability, in areas of care planning and care co-ordination;
  • A comprehensive understanding of the physical and emotional issues experienced by the elderly, people with a disability and their carers;
  • Strong communication skills, with a commitment to high quality customer service and ability to work with a range of stakeholders;
  • Well-developed computer skills and familiarity with Microsoft Office suite;
  • Current Victorian Driver’s Licence together with a reliable and insured vehicle;
  • Previous experience within a CHSP/HACC (or other funding) environment desirable and/or familiarity with the community services network and community-based care.
