First Call Officer
Allied & Community Health
Multiple Positions Available – Permanent Part Time / Fixed Term Part Time / Casual
‘First Call’ is an integrated, patient-centred intake and referral service that provides a central point of entry to all West Gippsland Healthcare Group sub-acute and Allied & Community Health services. This patient-centred central access model makes it easier for new clients to navigate our services. The First Call Officer will act as the first point of contact for our clients, completing initial needs identification and intake, and will provide information and assist with referrals to other services as needed. This role may be suited to enrolled nurses, welfare workers or those currently working in intake and referral roles.
Our ideal candidate will have the following qualifications and experience:
Mandatory:
- Good communication and engagement skills.
- Well-developed computer knowledge and skills.
- Experience in a healthcare/welfare/community services environment and a good understanding of the provision of associated services.
- A good knowledge of the range of issues for which people present in a community health setting.
Experience in working in a multi-disciplinary team.
Desirable:
- Experience with IPM/Uniti/S2S.
- Understanding, knowledge and experience implementing service coordination practices and protocols.
- Experience in undertaking initial needs assessment in a holistic and sensitive manner.
- Driver’s licence and use of own vehicle may be required from time to time.
