What you'll be doing The Manager, Aboriginal Health works with the Integrated Care Service (ICS) and is responsible for the management of the Aboriginal Chronic Care Program (ACCP) and Aboriginal Health Workers (AHW) to provide a culturally and clinically effective approach to Closing the Gap in preventable health conditions to achieve improved health outcomes for Aboriginal and Torres Strait Islander people in custody. The Integrated Care Service aims to support and enhance the existing systems to provide an integrated model of care for adult patients in custody and on release into the community with chronic or complex health conditions. Furthermore, ICS is to monitor patients from entry at reception through custody and post release unto the community, ensuring they receive seamless, timely, appropriate and effective health care. The Manager, Aboriginal Health will work collaboratively with the multidisciplinary team across JH&FMHN to ensure service delivery, monitoring and reporting of the Aboriginal Health Services. Applicants must be of Aboriginal descent through parentage, identification as being Aboriginal and accepted in the community as such. Aboriginality Is a genuine occupational qualification and is authorised under Section 14(d) of the Anti-discrimination Act 1997 A tertiary qualification in a relevant field, or a combination of study and relevant work experience Proven experience in ability to analyse data, produce reports and assess and manage data quality within agreed timeframes Proven experience to lead and manage a multidisciplinary team, work autonomously and form collaborative working relationships with internal and external stakeholders and agencies Proven high level written, verbal communication and negotiation skills, including the preparation of reports, submissions and business cases Demonstrated experience designing, implementing, and evaluating health strategies and initiatives to improve Aboriginal Health models of care Demonstrated experience in using Microsoft Applications, computer systems and data management Current unrestricted NSW Driver's licence and willingness to travel across the state as required Need more information?
