Social Worker - Community - Gastroenterology Employment Type: Temporary Part Time Position Classification: Social Worker Level 1 - 2 Remuneration:$30.85 - $44.69 per hour Hours Per Week: 32 Requisition ID: REQ52265 The CCLHD Social Work Department is seeking an experienced Level 1-2 Social Worker to join the team at Gosford Hospital. This position is responsible for providing a professional social work service to patients of the Liver Clinic and Gastroenterology Clinics, primarily seeing patients with diagnoses of long – term chronic digestive system disorders, such as hepatitis related liver disease and inflammatory bowel disease. This is a temporary part time position of 32 hours per week until 14th July 2019. What you'll be doing To provide a professional social work service to patients and their family/carers to ensure the provision of optimal health outcomes for patients. The position is currently based with the Gastroenterology and Liver Clinic at Gosford Hospital and provides a community service approach seeing patients and their families across the Central Coast. The worker will also liaise with ward based social workers when outpatients are admitted and sees patients on the wards when deemed appropriate for their ongoing care This position will work within a dedicated interdisciplinary team to deliver high quality care in a dynamic team environment Primarily the position is to provide assessment and counselling to assist patients and their carers to deal with the emotional impact of illness and to undertake case planning, liaison and advocacy and referral to ensure access to appropriate community services. The social worker is to work closely with interdisciplinary colleagues and senior managers to ensure the development and delivery of effective and pro-active social work services. The social worker will be expected to be active in the areas of service development, service evaluation and staff and student teaching. Qualification of a Degree in Social Work from an accredited course providing eligibility for membership of the Australian Association of Social Workers (AASW) (overseas qualifications must supply the AASW Overseas Assessment number) with demonstrated understanding of the current AASW Practice Standards as they relate to the position and a commitment to maintaining a professional skill and knowledge base. Previous experience in providing social work services in a health setting, preferably in the area of gastroenterology. A well developed understanding of the psycho-social impact of these health conditions and in the range of related social work interventions required in community social work clinical practice. Demonstrated effective verbal, written and interpersonal communication skills including the ability to adapt communication style to suit different audiences . Demonstrated ability to analyse problems, set priorities, apply a range of options to develop solutions and demonstrated experience in leading, conducting and evaluating quality activities. Demonstrated commitment to professional development, evidence based practice and continuous improvement. Demonstrated ability to work as a member of an interdisciplinary team. Demonstrated understanding of how the District is Caring for the Coast and how this role would contribute. Need more information?
