Our Health Fund operations include Queensland Country Care Navigation, a telephonic Care Navigation services to deliver person-centred, seamless, efficient and effective care, particularly for people recently discharged from hospital and those with complex, long term conditions.
  
The Role
  
As the Care Coordinator you will work as part of a team led by the Care Navigation Manager to identify, contact, plan and implement a post hospital discharge program to support Queensland Country Health Fund Members to transition home safely. The telephonic Care Navigation service provides direct or indirect, best practice clinical care and education through coordination of services and education programs.
  
This is a Full Time salaried position based on a minimum 38 hour working week; however the successful applicant must be prepared to work outside of these hours if required.
  
Selection Criteria:
  
  • Registered Nurse with current AHPRA registration
  • Solid nursing background in a variety of clinical areas (highly desirable) or experience working with clinical services in the community
  • Experience working as part of a team
  • Demonstrated flexibility, strong problem solving skills and an openness to new ideas
  • Strong interpersonal and communication skills
  • Intermediate computer skills
  • Knowledge of the types of support that may benefit members experiencing chronic disease, mental health concerns or in the recovery and rehabilitation post joint surgery (highly desirable)
  • Demonstrated experience in delivery of care coordinated programs or a background in coordinated discharge programs (desirable)
  • Experience in population health or health education (desirable)
