Where you'll be working Liverpool Hospital is the major health service for South Western Sydney providing services to the local government area of Liverpool City Council as well as district services to residents and visitors in the area. It also provides a range of statewide services in areas such as critical care and trauma, neonatal intensive care and brain injury rehabilitation. We are situated in one of the fastest-growing regions in Australia and sit within an education and health precinct which includes the Ingham Institute of Applied Medical Research, a Clinical Skills and Simulation Centre, the Clinical Schools of the University of NSW and University of Western Sydney, Sydney South West. What you'll be doing The Case Manager is responsible for working collaboratively with clients of the Brain Injury Rehabilitation Unit (BIRU), their families, multidisciplinary teams (MDT) and others to aid recovery, resettlement and social re-integration into the community. This involves the coordination of the multidisciplinary therapy team and other relevant services, government and non-government, to implement client centered rehabilitation programs. The Case Manager is also responsible for other tasks necessary to support service delivery including data management, quality improvement activities, professional development, and team projects and planning. Selection Criteria Approved iCare case manager or willing to apply for iCare accreditation Experience in Case Management of clients with brain injury or clients in a rehabilitation setting. Excellent time management skills and ability to work independently and as part of a multidisciplinary team (MDT), including the coordination of a MDT. Demonstrated excellent verbal and written communication skills including interpersonal negotiation and conflict resolution skills, and computer literacy in a range of programs. Demonstrated experience working with, or understanding of, external funding bodies such as iCare/CTP/workers compensation/NDIS, and their guidelines and processes. Demonstrated understanding of and/or experience participating in quality improvement activities and implementation of evidence based practice.
