ABOUT THE ROLE:
The Health Independence Program (HIP) at Werribee Mercy Hospital specialises in best practice in-centre and home based programs to help clients living within the Wyndham catchment improve their health and achieve their rehabilitation goals. HIP consists of five services, including Community Based Rehabilitation, Specialist clinics: Falls and Balance and Continence, Residential InReach and Complex Care.
The Complex Care team offers short term care coordination and goal directed nursing and allied health intervention to help clients reach their full potential and continue living independently. Within the role there is the opportunity to facilitate Pulmonary Rehabilitation program, which has a strong multidisciplinary approach to promoting exercise, quality of life and disease self management. This is a high performing team working Monday – Friday office hours, weekends and public holidays off.
- 0.6EFT until 6 September 2019
- Deliver centre and home based physiotherapy services to a diverse client group with complex health and social welfare needs.
- Improve self –management skills of clients with chronic diseases
- Expertise in respiratory physiotherapy preferable
- Use of motivational interviewing techniques to help clients explore and resolve their uncertainty
ABOUT YOU:
- Recognised Degree in Physiotherapy, current, full physiotherapy registration with AHPRA
- Current Victorian (or equivalent) Drivers Licence
- Strong, relevant clinical skills and experience in sub-acute/ community setting
- Highly developed communication and interpersonal skills,
- Experience and knowledge in the management of people with complex chronic care needs
- Commitment to person-centred care and ability to adapt and learn quickly.
- An ability to work autonomously and flexibly within an inter-disciplinary team

Werribee Mercy Hospital is a General Public Hospital located in Wyndham LGA. Our services include surgical, medical, maternity, newborn, renal dialysis, emergency, mental health, rehabilitation, aged and palliative care. The hospital is currently undergoing significant expansion with a number of exciting capital projects currently underway, including an $85 million State Government funded redevelopment that will deliver a new critical care unit and take the total number of beds to 298.
