The Role:
Key Change Management Role
We have an exciting opportunity for an experienced integrated health care professional to join our team, based in either Nowra or Wollongong.
As a senior member of the Engagement and Coordination Team, you will be developing and implementing innovative programs that are strategically aligned and responsive to community needs.
Key focus areas include:
- Forging strong relationships with the Illawarra Shoalhaven Local Health District and working collaboratively on shared priorities
- Facilitate initiatives to improve the patient journey into and from tertiary care settings
- Identifying appropriate system integration initiatives such as service redesign and hospital avoidance strategies
- Overseeing the implementation of Illawarra Shoalhaven HealthPathways program
