An exciting opportunity is now available for a Care Coordinator Team Leader to work on a full-time basis, across our Kalgoorlie site. 

Role Overview

360 Health + Community operates a multidisciplinary health service, delivering services for patients living with a chronic condition or identified as at risk of developing a chronic condition. The organisation is expanding both geographically and in terms of the breadth of our clinical services offered.

The Care Coordinator Team Leader will coordinate the care and referral pathway for patients. This will include the triage of referrals, individual assessments of patients and coordinating the clinical activities and/or services required. Duties will include the provision of clinical management, care coordination and supporting general practices accessing the service.

This role will provide support to other members of the team in the delivery of services for patients with cardiovascular disease, type 2 diabetes, chronic renal disease, chronic musculoskeletal conditions, and chronic respiratory disease.

 

Essential Minimum Selection Criteria

Relevant Registrations

  • APHRA registration

Experience/Qualifications

  • Good knowledge of chronic disease management and prevention.
  • Clinical experience in case management of patients with cardiovascular disease, type 2 diabetes, chronic renal disease, chronic musculoskeletal conditions and chronic respiratory disease.
  • Experience and qualifications as a cardiac, respiratory or renal nurse will be highly regarded

Skills, Abilities and Knowledge

  • Good knowledge of referral management and practice management systems.
  • Good knowledge of programs available within the primary care setting.
  • Extensive knowledge of primary care and the use of chronic disease Medicare item numbers.
  • Demonstrated ability to assess client needs within scope of service delivery.
  • Ability to work to deadlines and be structured in allocation of time and resources.
  • Working knowledge and ability to use IT and clinical software programs for patient management and reporting requirements.
  • Ability to contribute to written reports in a concise, professional and appropriate manner.
  • Ability to work both independently and collaboratively with other members of the team.
  • Ability to maintain accurate records that reflect best practice in service delivery in accordance with 360 clinical governance framework.
  • High level of interpersonal and communication skills, and the ability to maintain confidentiality.
  • Diplomacy and tact in dealing with all stakeholders.
