The Community & Home Care Services Team are seeking a part time Home Care Manager with ;
Essential:
- Allied Health Professional or Registered Nurse with current Registration under AHPRA (Australian Health Practitioners Regulation Agency) or self-regulation.
- Minimum 3 years clinical practice in the Health, Aged Care or Community Home Care Sectors
- Demonstrated skills and experience in conducting comprehensive assessments and development of goal oriented care plans
- Proven experience in leading teams and contributing positively to the workplace and culture
- Knowledge and experience working with people living with dementia
- Experience in optimising carer and social support programs
- Advanced written and verbal communication skills
- Proven commitment to and application of consumer directed care
- Current First Aid Certificate (ongoing requirement)
- Comprehensive experience in working in a flexible, multi-skilled team environment with advanced interpersonal, communication, customer service and networking skills.
- Current drivers licence
As part of the recruitment process candidates will be required to undertake a functional assessment including drug and alcohol testing.
