To be the successful Care Manager for this position, you will have comprehensive knowledge of the Aged Care sector and demonstrate experience in management, including comprehensive assessment, planning, provision and evaluation of services for frail aged residents with disabilities and dementia.
You will be able to work independently and professionally with limited supervision. You will also be sensitive to and have experience in multicultural settings, and possess the ability to work within a diverse community.
Requirements
- Demonstrate comprehensive knowledge of ACFI funding and the Aged Care policy protocols and processes.
- Show an ability to operate independently and professionally with limited supervision.
- Interpret work within the relevant policy guidelines
- Understand the needs of older people, in particular those with cognitive decline
- Work autonomously as part of a team
- Coordinate and supervise the clinical staff
- Tertiary qualifications in Health, Welfare, Nursing or Social Sciences
- Experience in coordination and supervision of clinical staff
- Experience in Aged Care accreditation and the clinical standards, dementia and challenging behaviours
- Experience in working closely with multidisciplinary teams
- High level of computer literacy with the ability to navigate and input data management systems
