Full time permanent position - Napranum
This position will work with the Females of Napranum to maximise health outcomes by contributing to the provision of a culturally appropriately primary health care and chronic conditions management service, as a member of a multi-disciplinary team, delivering services consistent with the Apunipima Model of Care and within a remote comprehensive primary health care context.
The successful candidate will hold Certificate IV Aboriginal and Torres Strait Islander Primary Health Care (Practice) and Health Practitioner registration with AHPRA - essential. Previous demonstrated experience and competency as a Health Worker within a clinical practice environment, with a sound understanding of integrated and collaborative client care within a multi-disciplinary team and demonstrated experience working collaboratively with others to facilitate a quality comprehensive primary health care service for clients.
