About the company

This provider is a nationwide healthcare NFP organisation with hospitals, residential aged care homes and community care services. This role falls under the residential and home care entity who are responsible for delivering a range of services from aged care and disability as well as other support services for the community.
About the opportunity
You will be responsible for providing nursing services to clients in collaboration with case managers and the internal team of the organisation. You will assess, plan, implement and evaluate your clients nursing care with specific focus on the maintenance of health and prevention of illness, support with pain management and wound management.
You will report directly to the Area Manager and ideally you will have your own car (there are pool cars available however they are shared across the staff).
This role is primarily focused on the care of clients who have come from a hospital setting and moved back into their home. Therefore, experience working within the community or within an acute hospital setting is essential.
Duties and Responsibilities
- Provide acute clinical care services for clients across the Adelaide area
- Work closely with clients and families, liaising with the wider community to identify clinical goals
- Continue improvement of quality care through assessments, planning, implementation and evaluation
- Responsible for mentoring and educating care workers and EEN’s
Skills and Experience:
- Tertiary qualifications in Clinical, Allied Health, Social Science or a related field
- AHPRA Registered Nurse Div 1/Div 2 with at least two years recent community or acute nursing experience
- Current unrestricted Drivers Licence and willing to travel
- Strong understanding of clinical guidelines
- Excellent written and verbal communication
- Ability to network within the community and engage external and internal stakeholders
- Ability to play a ‘hands on’ role within the organisation
