What you'll be doing The Clinical Nurse Specialist Grade 2 (CNS2), Acute Respiratory Liaison is responsible for the provision of safe high quality patient centred nursing care to patients experiencing an acute respiratory illness with a particular focus on those with a diagnosis of Chronic Obstructive Pulmonary Disease (COPD) at Wyong Hospital. This outcome will be achieved through the exercise of advanced nursing skills, professional knowledge and judgement to provide a high standard of assessment, clinical care, expert advice and educational support to staff, patients, family and carers. Consistent with the strategic directions of Central Coast Local Health District (CCLHD) ‘Caring for the Coast’ and service plans, the CNS2 has a leadership role in the development, provision of evidence based nursing clinical practice related to acute respiratory. While working collaboratively with the multidisciplinary team, the CNS2 will coordinate the care and follow up for patients who have been diagnosed with an acute respiratory illness, particularly those patients with an acute exacerbation of COPD to allow for earlier discharge from both the Emergency Department or inpatient wards and reduce the risk of representation. As an employee of CCLHD, you will be rostered in accordance with the Nurses and Midwives (State) Award and in line with the operational requirements of your unit/ward/service. Current registration with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse (Division 1) with post registration qualification relevant to acute respiratory nursing and at least 3 years’ recent experience working in the acute clinical area of the post graduate qualification. Demonstrated experience utilising knowledge and advanced nursing skills in the assessment, care planning and provision of safe clinical care for patients with needs related to acute respiratory illnesses and COPD. Demonstrated commitment to quality improvement through the use of evidence to evaluate care and implement change. Demonstrated written and verbal communication skills, computer literacy and capacity to use health related information technology systems. Demonstrated ability to communicate effectively and contribute nursing expertise to the improvement of care within a multidisciplinary care team. Demonstrated commitment to professional development as evidenced by the provision of clinical advice, teaching and leadership.
