The Clinical Nurse Consultant is accountable for the effective clinical nursing consultancy of clients within diabetes, education and management.

You will have a well-developed understanding of the ageing process and common interventions required in the care of acute, chronic and complex care needs of consumers.

 
About your role


  • Accept client referral within the local area and specialist teams
  • Provide consultation, expert assessment and care planning to local area team members
  • Proactively provide clinical leadership to local area care teams within the region
  • Work in collaboration with Senior Clinical Nurse Advisors to develop, maintain, review and promote best practice standards within the organisation
  • Establish positive professional practice relationships with other health practitioners and support staff
  • Facilitate the development of clinical expertise in area of specialty within the region
  • Identify clinical learning needs of local area team members and facilitate learning opportunities
  • Forge and maintain professional networks and relationships with external stakeholders


What do we need from you?

  • Advanced clinical knowledge in diabetes
  • Must be available 8 hours per day, Mondays & Fridays (preferable but days are flexible)
  • Demonstrates a professional and person centred approach to the provision of care, including an understanding of the principles of wellness and reablement for older people.
  • Recent clinical or managerial experience in assessing, planning, implementing, and evaluating acute, chronic and complex client care needs of community based clients
  • Demonstrates a well developed range of clinical assessment skills, in a range of situations relevant to the care needs of the cohort of clients.
  • Have well-developed negotiation, interpersonal, communication and education skills
  • Proven ability to work autonomously and as a part of the multidisciplinary team environment
  • Possess a well developed ability to manage priorities and work within limited timeframe


Qualifications:

  • Registered Nurse (Division 1)
  • A current registration with AHPRA
  • Post Graduate Qualification in a relevant specialty - diabetes education & management
  • Current and valid police check (valid within the last 3 years)
  • Current and valid Australian Drivers licence and access to a car for work purposes

Nice to have:

  • Post Graduate qualification in education and/or Certificate IV in Workplace Training and Assessment
