Nexus Primary Health has recently been funded through the Eastern Melbourne Primary Health Network for innovative after-hours and diabetes diversion projects. Both of these projects are about changing service delivery to better meet the needs of our communities.
We have a variety of roles available including this senior position of Integrative Diabetes Coordinator which is part of the Diabetes Diversion Partnership Project. There is a substantive project management, planning and coaching aspect to this position.

Let's talk about the role
The Integrative Diabetes Coordinator manages the Diabetes Diversion Partnership Project across three sites at Wallan, Kinglake and South Morang. The role requires sound project planning and project management to ensure that negotiated and reasonable deadlines are delivered. The Coordinator provides the vision and direction to a range of partners and stakeholders to bring about change and provide superior holistic client-centred diabetes services.
The Coordinator also needs a high level of credibility in the Health sector as they will be dealing with a range of clinicians, nursing staff and Allied Health professionals
Here is an opportunity for you to build a first class diabetes care model across several health centres in the project catchment areas.
Let's talk about you
You have solid project management and coaching qualifications/experience. You have a collaborative style of communication, working across all levels of health professionals. You'll draw upon your tertiary qualifications in health sciences, nursing or chronic disease with an understanding of diabetes principles and practices.
