The Bolton Clarke Homeless Persons Program actively seeks to engage and offer a holistic primary health care response to individuals and families who are at risk of or who are experiencing homelessness. The program is committed to offering practical health care at the point of contact and actively works with people to link them into generalist and specialist services.

This position is collocated with The Salvation Army Crisis Services - Access Health St Kilda and is an assertive outreach position within the City of Port Phillip. The position will also include occasional coverage in a nursing clinic at Access Health.


We are seeking applications from nurses who can exercise a high level of professional autonomy and responsibility in a community setting. Applicants should have a strong interest in primary health care and a commitment to improving the health of homeless and disadvantaged people.

The ideal candidate will have the following: 

• At least 5 years Post Graduate experience
• A strong interest in primary health care
• Capability of exercising a high level of professional autonomy and responsibility in a community setting
• Ability to form and maintain strong collegial relationships across service organisations
• Experience or interest in working with complex clients
• Ability to translate existing skills within a new environment
• Commitment to improving the health and service outcome for people who are marginalised
• All candidates must have a current Victorian Drivers Licence and APHRA registration

Whilst the position is co-located with another service, the Homeless Persons Program maintains a strong team focus, supported by team meetings, shared educational opportunities and formal and collegial support strategies. HPP will provide one month orientation, weekly formal support and monthly external supervision.
